This form is on PHP/HTML. It has php validation. E-mail messages are sent to multiple email addresses with the data entered and a confirmation message to existing or future customer.
This form does not insert the data into a database however.
This form is completely customizable. You can add or delete form fields by simply changing one line. You can also create the form with one of the scripts in the package and copy-n-paste it in an existing or new web page. A PHP form manager will then validate and process the input data.
You can use this for free as long as you comply with our terms or pay $25.00 to remove our link in the form.
You need to contact us if you need further customization. But, this is unlikely since most fields corresponding to a contact form are already functional (first name, last name, phone, email, company, subject, message, etc).

You do not have to give out your contact, support or sales E-mail addresses. You can change a few fields based on your criteria to filter the new coming messages from older ones. For example, putting the date on the subject, or the city, etc.

TEXT_SIZE:N : N is a number for the exact size.
TEXT_RANGE:N:N : N is a number for size range. The first N is minimum and second is maximum.
NUMBER_SIZE:N : N is a number for size.
NUMBER_RANGE:N:N : N is a number. The first N is minimum and second is maximum.
E-MAIL : Email address field.
URL : Web site address field.
You can very easily add new formats.
Customizable Form Validation Error Reporting:
ALERT_WINDOW - Display error messages in an alert window and automatically go back to the form.
FORM_PAGE - Display error messages in the same form the user completed and even display a picture on each incorrect value.
NEXT_PAGE - Display error messages in another web page with a link to Go Back and fix the errors in the form.
POPUP_WINDOW - Display errors messages in a popup web page and keep the user in the form.
Customizable Form Data E-mail Delivery:
Form data can be sent either in the message body or as an attachment in the following formats:
HTML - Web page format
XML - XML format with each field as a node.
CVS - Comma Separated File for Easy Spreadsheet Importing.
Any Other Delimiter - You can also choose any delimiter you wish.
